One of the most common questions asked is if oral surgery is safe during pregnancy. Naturally patients are concerned about the health of their baby and do not want to risk anything during pregnancy. This is a good question, and always something to discuss should you need oral surgery and you are pregnant.

 

Typically oral surgery is not recommended when a patient is pregnant, and we feel protecting your child’s health is extremely important. With pregnant patients we avoid general anesthetic as well as sedation, and will only use local anesthetic. A recent study in 2015 by the American Dental Association found that local anesthetics do not harm an unborn child’s health.

Does It Matter What Trimester I Am In?

Another factor we consider is how far along the patient is with their pregnancy. Usually first and second trimesters are safe for oral surgery that does not require the patient to be sedated. We avoid any procedures during the third trimester because any surgery when pregnant is risky during this period.

 

If you are in need or oral surgery our team works closely with you to determine what is really medically necessary, and we recommend postponing the procedure until after the baby is born.

Should Oral Surgery Be Done Before Pregnancy?

We always recommend if a patient is actively trying to get pregnant, they should visit our office to ensure you have no issues. This way we know if any issues need attention right away, and what can wait until after the baby is born. We encourage our patients to always take care of any oral surgery as soon as possible, so you do not have to worry during your pregnancy.

Should Oral Surgery Be Postponed Until After Child Birth?

If you are in need of oral surgery and are pregnant we always recommend waiting until after child birth. The only time oral surgery is recommended is when the mother’s health is in danger. During the third trimester we always avoid any surgeries and take every possible precaution. You and your child’s life and health is our priority. Most oral surgeries can be postponed, and there is typically no immediate need to perform surgery during pregnancy.

 

We recommend patients always come in for their regular exam and cleaning even during pregnancy so we can make sure all looks and feels great. During your exam we may use x-rays to take a closer look in your mouth and jaw. X-rays are safe for pregnant patients, and are at a low radiation level where there is no risk of harm to a developing baby. During any x-rays we ask our patients to wear a shield, and this provides an extra level of protection.

Root Canal

Root canals are a common procedure and have a success rate of 95%. There are procedures in place that can help your root canal last such as placing the crown immediately following the root canal. It is also important to practice good oral hygiene habits after your root canal at home.

 

There are some cases where patients do have an issue after their root canal is complete. Even though it is unlikely, root canals can fail. Failed root canals can be due to cracks in or around the root area, or from an obstruction that can make cleaning the area difficult.

 

There are symptoms you can look out for that may indicate you may have an issue. These symptoms include sensitivity to hot or cold, swelling, or pain while chewing. If you experience any of these symptoms we urge you to call our office as soon as possible.

How Does an Apicoectomy Work?

If a patient has a blockage around the tooth that is preventing access to the root canal they may need an apicoectomy procedure. This procedure involves creating a small flap in the gum to access the root. The infected area is cleaned out and sealed to prevent further infection. This is a great way for our team to access the root area that has the blockage and save your tooth.

Other Treatment Options

Retreatment

The most common option for failed root canals is retreatment. This involves removing the original filling, and cleaning and disinfecting the canal. The area is then resealed to prevent further infection. This treatment has the highest success rate.

Extraction

If both the apicoectomy and retreatment are not effective, removing the tooth is the last resort. With this option dental implants can be used to fill the empty space where the tooth was extracted.

A normal bite is when both the upper and lower jaws align perfectly. The upper teeth overlap the lower teeth, and the jaw functions normally. If alignment is off this causes a malocclusion or issue with your bite. With an underbite the lower jaw protrudes further out than the upper jaw.

What Can Cause an Underbite?

An underbite can be caused by a child using a pacifier or thumb sucking. Most underbites are genetic and run in families. Irregularities in jaw size and shape cannot be prevented and require professional help.

 

An underbite can cause issues with chewing or speech, and can even cause decay. Tooth enamel can be lost where the teeth overlap, and jaw pain is common with patients that have an underbite. We want our patients to feel confident about their smile, and we are always happy to help each patient get that smile they have always wanted.

How Can My Orthodontist Help?

Orthodontic treatment for an underbite is common. Part of our detailed consultation process is to take a detailed look at your mouth, teeth, and skeletal structure. Depending on the severity of the underbite will determine the treatment plan. We look at severity, the patient’s age, and potential cause of the underbite.

 

If your child shows signs of an underbite, we recommend parents bring their child in by the age of seven. Early intervention is always best, and allows our team to get in front of any underbite issues at an early age.

 

In many cases braces can help reposition the lower teeth. Dental extractions may also be necessary to make room. Other appliances such as expanders can be used to help correct bone development. Unfortunately, braces and appliances are typically not enough for every patient that has an underbite.

When Is Surgery Necessary?

An oral surgeon can help correct an underbite by reshaping and repositioning the jawbone. This type of surgery is called orthoganic surgery. This surgery can help to lengthen or shorten the lower jaw depending on the severity. In some cases bone may need to be added or removed. Small plates or screws are used if necessary to stabilize the bone after surgery.


Surgery is usually recommended to correct an underbite for patients that are in their late teens or adults, and their jaw bones are already fully formed.

These implants create a base for one or many artificial teeth or crowns that are affixed on top. During the implant process healing time is needed for the implant to fuse to the jawbone. This type of implant is durable, long lasting, and a popular choice.

Subperiosteal Implants

Subperiosteal implants are a good alternative to endosteal implants. This type of implant sits on top of the jawbone, but is still under the gum line. Using a metal frame the post is attached and the gum then heals around the frame. This allows the implant to be held securely in place. Artificial teeth or crowns are then placed on top of the poles that are in the gum area. In many cases this type of implant is used if patients do not have enough jawbone to support endosteal implants. Bone grafting can be done to provide the additional bone needed, but some patients choose to not go through that process.

What Are My Alternatives?

If a patient’s jawbone cannot support dental implants there are techniques available to help rebuild the jawbone and create a stable foundation for implants.

  • Bone Augmentation – This procedure helps to restore or regenerate bone in the jaw when a patient does not have enough to support an implant. This process is very successful, and many patients choose this process in order to get those implants they have always wanted.
  • Sinus Lift – A sinus lift can add bone below the sinus cavity to help that area if bone has deteriorated.
  • Ridge Expansion – Bone grafting material can be added along the top of the jaw or small ridge to help make your jaw wide enough to support dental implants.

What Are Other Dental Implant Options?

  • Mini Dental Implants – Mini dental implants are similar to traditional dental implants, but they are used for smaller spaces. Patients that have naturally smaller teeth may find that mini dental implants fit them best. This procedure is less invasive and tends to be less expensive compared to other options. Mini dental implants are made of one solid piece that is screwed into the jawbone. Each implant has a ball on top and this allows the crown to be secured.
  • Immediate Load Dental Implants or Teeth In a Day – This type of implant process allows the placement of the crown or temporary tooth at the same time as the dental implant. Patients that have sufficient bone structure in the jaw are good candidates for this option.
  • All-On-4 Implants – All-on-4 implants provide patients with implants in just one appointment. With this option implants are not required for each tooth. Four implants are placed, and this makes the process easier and less invasive. Bone grafting is not needed with this option, and temporary teeth can be placed that same day. With this option there is a healing process while the implants fuse to the bone. Once the healing process is complete, permanent teeth can be placed.
